Why does my site go out of whack on a smart phone?

Hey folks..

Some elements of my site are messed up when looking at the site on a smart phone..

Will this generally happen?

Do I have to recode the whole site to be compatable with smartphones?

Do I need two seperate versions of my website? One for desktop browsers and one for mobile?

Bit of info,

I've used fixed width's all the way through

a picture protrudes from my container on the right, and also some text looks like it's been boxed in a smaller width div.
