Problem with my form

Hi All

My first post so hello and thank you for any help you manage to give me.

I have built a website that is working OK but I have one small issue, I have a form collecting data for the import export of vehicles from Southern Africa. When I try to put my cursor in a box it jumps several boxes up, the only way I can get it to work is by using the tab key which is not obviously good if my customers don't know this. I am pretty new to web building but can someone look at my code which was built in Dreamweaver and tell what I have done.

Use submit button with declare name , action , destination properly to get it solved
