Creating a form that will give an estimate/quote

Hey guys, I am trying to figure out how to go about make a form I guess, that will allow me to have several options to be chosen to be added up and give a final "quote" as in a final price or an estimated price of the services offered in the options.


I figure PHP or asp would be used to handle something like this.
