creating registration form for payment of product.

dear members i need help regarding creation of page which is Registration page for user to purchase product..

we see in some sites like Order Now / Buy Now. When we click there it redirects to some other page and form will be seen out.

such a page is required for me. kindly tell me which script is essential for this output desired. :idea:
