Install Page

Hi......

need some help please..... :-)


I have a folder with 3 PHP files in:

1) index.php 2) about.php 3) input.php

what i need is an install page that can create the mysql db without the person messing with there server directly..

info for the db to receive
1) Name 2) Email 3) domain Name 4) site info

could someone help please
