Website form to send Attachments to Email

I am looking for a simple php script to created a website form which will ask the client to fill out certain fields which will also allow them to send an attachment.

The attachment will goto an email address so that Outlook will see both the client details and also the attachment within the email.

I.E

First Name
Second Name
Email
Send CV/Resume (browse button)
CAPTCHA
(SUBMIT BUTTON)
