Apache link to other devices on LAN ?

Hi All!
I'm new here this is my very first post.
I'm also a total n00b so be warned. :)

Can some one please point me in the right direction to do the following.

I have a number of devices on my LAN like DD-WRT routers etc.

On my Apache web server I want to have a web page that can re-direct to some of these other devices to show the info pages etc.

How do I go about doing this?

My preference would be to use PHP, or raw HTML.

I'm not sure how, but then I'm also aware of the potential security risk. I'm assuming that you run your server from home? I would not want any of my other devices going anywhere near my server, at all.
