Feedback application

Can you recommend some simple application for collecting feedback from website visitors? I would like to put it in my website and get visitors opinions on how my website works. I couldn`t find anything interesting yet .

You could use Cbox which is a simplified comments code:-

or use Wordpress which is blogging software that you can integrate into your web page, slimming it down so that it has just one simple comments box.

Both the above enable viewers to see the comments, but if you want private comments use a form which sends comments to your email address:-

or to a database:-
