Linking a form to MySQL data

I would like to have a page on my football club's website that lists all the players from the MySQL database on the left for example and then a form on the right of it which will populate information from the database with whatever player is selected.

Is this easy enough to do, and if so, does anyone have any tips or links to good resources?

Has anyone got any ideas or if not, any links to good tutorials which explain how to link MySQL to a web page?
